Investigators from the U.S. Department of Veterans Affairs' Office of the Inspector General found that the unit's medication refrigerator was frequently kept at incorrect temperatures, medication carts did not always locked properly and medical supplies were not adequately stocked.
No instances of inappropriate patient care or harm were identified during the June visit, and the medical center developed a plan for correcting the deficiencies.
